The GriT Program, under AMD Care Services, is a personalized support system designed to empower individuals to achieve their full potential and live fulfilling lives. GriT focuses on fostering independence and community integration by partnering with clients and their support networks to identify and prioritize essential life skills, personal choices, and individual motivators. The program offers tailored pathways to success through a range of resources, including education, vocational training, employment preparation, and other life skills development. The ultimate aim of the GriT Program is to support clients in achieving their desired level of independence and building a rich and meaningful life, whether that involves living independently or with family and friends.
We are providing the following services:
The Community Adult Day Program offers a structured and supportive environment five days a week, Monday through Friday, for six hours each day. With a staff-to-client ratio of 1:3, the program focuses on community integration, life skills development, and vocational training.
Tailored Day Services offers flexible support, scheduled by the client or their family, any day and time of the week. These services are provided on a one-to-one basis for 11-12 hours weekly, with staff-provided transportation. All activities take place within the community, utilizing available resources. The program focuses on areas such as education, volunteering, job preparation, life skills, mobility, social skills, and safety.
The Paid Internship Program (PIP) is a one-on-one service that offers clients the choice between instructor-led support or an independent internship experience. Clients utilize approved hours from the Regional Center to engage in practical, vocational training within a real job setting. The primary goal of PIP is to equip individuals with the necessary skills and training to secure competitive employment. This service requires authorization from the Regional Center, mandates that clients earn at least minimum wage, and is available to individuals aged 18-59.
Community Integration Training (CIT) provides one-on-one support for clients who are unable to participate in traditional day programs. This individualized attention addresses a variety of needs, including daily living skills, employment goals (such as increasing work hours), educational pursuits, and other areas as authorized by the Regional Center, with no set limit on hours. Clients can receive up to six hours of daily support, five days a week. CIT is particularly beneficial for individuals with behavioral challenges. Transportation is provided, and all services are delivered in the community for individuals aged 18-59.
